This form is set up to automatically change the designation and email notification based on the month of the year as calculated in the Month Name field. The FALL and SPRING field Sections are exposed based on the Month Name field values, but for easier conditional rule form formula building, use the  "Spring: Yes" or "Fall: Yes" fields, as is done in the Email Notification formula.
